In e-cigarettes, tobacco combustion is replaced by e-liquid heating, leading some manufacturers to propose that e-cigarettes have less harmful respiratory effects than tobacco consumption. Respondents who perceived a lower level of local retailers’ compliance to the ban were more likely to obtain e-cigarettes from illegal sellers and mixed flavored e-liquids on their own, compared with those who perceived a higher level of local retailer compliance. A possible explanation is the neighborhood effect, that is, in neighborhoods where law enforcement is relatively weak, local retailers are less likely to follow the ban and illegal sellers are more active.

Notably, the reverse was not true—students who said they smoked cigarettes were no more likely to report use of e-cigarettes when asked approximately 6 months later. Electronic cigarettes, or e-cigarettes, are tobacco products that have been sold in the U.S. for about a decade. Several studies have also suggested that nicotine is potentially carcinogenic (reviewed in [41]), but more work is needed to prove its carcinogenicity independently of the combustion products of tobacco [47]. After adjusting for these variables, e-cigarette users were 34 percent more likely to have a heart attack, 25 percent more likely to have coronary artery disease and 55 percent more likely to suffer from depression or anxiety, the release states. Stroke, high blood pressure and circulatory problems were no longer statistically different between the two groups.

Given their relatively recent introduction, there has been little time for a scientific body of evidence to develop on the health effects of e-cigarettes. E-cigarettes were developed and marketed as alternatives to traditional combustible cigarettes. However, e-cigarettes are not an FDA-approved quit aid, and there is no conclusive scientific evidence on the effectiveness of e-cigarettes for long-term smoking cessation. They have the potential to benefit adult smokers who are not pregnant if used as a complete substitute for regular cigarettes and other smoked tobacco products. However, E-cigarettes still contain nicotine, the chemical that makes traditional cigarettes addictive. They are not safe for youth, young adults, pregnant women, or adults who do not currently use tobacco products.

The majority of respondents currently smoked cigarettes either daily (30.9%) or weekly (44.4%). Most respondents had been aware of the ban before the survey (67.7%), with a plurality feeling neutral about the ban (36.1%) but more supporting the ban than opposing it (35.9% vs. 28.1%). Among respondents who knew about the ban before the survey, the perceived compliance of local retailers with the ban as lower than 50% was quite high (66.2%), and relatively few perceived that more than 75% of retailers were complying (14.8%). On Dec. 21, 2022, California enacted Senate Bill 793, which prohibited the sale of most flavored tobacco products, including e-cigarettes, to people of all ages. Hookahs lost vape ursa nano, premium cigars and loose-leaf tobacco were exempted from the legislation.

E-cigarette sales tripled between 2012 and 2013 rising from $500 million to over $1.5 billion and are expected to soon exceed the sales of traditional cigarettes, motivating giant tobacco and other non-cigarette companies to market this product. A popular claim is that nicotine in e-cigarettes is relatively harmless, whereas additives and combustion products largely account for the harms of traditional cigarettes. The UofL research, which tested the effects of e-cigarettes with various types and doses of nicotine in animal models, showed that the nicotine form contained in pod-based e-cigarettes — nicotine salts — led to heart arrhythmias, particularly at higher doses.

A study in the Journal of the American Medical Association in 2015 found a connection too. Researchers surveyed 2,500 Los Angeles high school students who had never smoked. They found that kids who used e-cigs were more likely than non-users to smoke cigarettes or other tobacco products over the next year.

ENDS emissions therefore pose potential risks to both users and non-users. Some vape shops may have legal responsibilities as both manufacturers and retailers of tobacco products. These products use an “e-liquid” that usually contains nicotine derived from tobacco, as well as flavorings, propylene glycol, vegetable glycerin, and other ingredients.

“EVP use is not a safer alternative to smoking but may have contributed to the decline in regular tobacco product use. EVP use also raises concerns about new health risks, including nicotine addiction.” Electronic vapor products (EVPs), also known as e-cigarettes or vaping devices, have an allure because of their marketed image as a safer alternative to traditional cigarette smoking and for their variety of appealing flavors. Other compounds that have been detected in aerosols include acetamide, a potential human carcinogen [5], and some aldehydes [69], although their levels were minimal. Interestingly, the existence of harmful concentrations of diethylene glycol, a known cytotoxic agent, in e-liquid aerosols is contentious with some studies detecting its presence [4, 68, 70,71,72], and others finding low subtoxic concentrations [73, 74].

Researchers are working hard to gather more information about e-cigarettes and how they are used. This information may lead to additional regulations and could be helpful for informing the public about what’s in e-cigarettes and the potential health risks of using them. Studies on e-cigarettes as a quitting aid for nicotine addiction and cigarette use are few, with mixed and modest results. Some show that they are about equal in effectiveness to that of nicotine replacement therapy (NRT) – like the nicotine patch. The best way to quit is to use quit line counseling and FDA-approved medications. E-cigarette use among both youth and young adults has increased considerably in recent years.

In response to this issue, the PACT Act Amendment prohibits sellers from using the U.S. Post Office to ship e-cigarettes, vapes, flavored and smokeless tobacco products. The PACT Act applies to all shipments in interstate commerce (15 U.S.C. § 376) as well as to all delivery sales. While e-cigarettes are often marketed as a way for adults to quit nicotine, a whole new generation of non-smokers is now taking to the habit in young adulthood.

In 2017, FDA extended the deadline for completed applications to August 2022. When several public health groups, including Truth Initiative, sued FDA, a federal court ordered the agency to require applications be submitted by May 2020. The deadline changed again to September 9, 2020, following FDA and tobacco industry requests for more time due to the COVID-19 pandemic. The potential public health impact of e-cigarettes is a subject of hot debate. Despite inconclusiveness about their short- and long-term health effects, e-cigarettes remain largely unregulated and their popularity among youth and young adults continues to rapidly escalate. At the same time, serious illness and deaths have been linked to their use and recent research could not conclusively determine that they posed fewer health risks than combustible cigarettes.

According to the CDC, 15% of EVALI patients were less than 18 years old. Brain development begins during the growth of the fetus in the womb and continues through childhood and to about age 25. Nicotine exposure during adolescence and young adulthood can cause addiction and harm the developing brain. Equally disturbing are two reported deaths and thousands who have been injured or burned because of “vape pen” explosions.

About one-quarter of U.S. youth and young adults have ever tried e-cigarettes. While vaping might help you quit smoking, it probably won’t help you quit nicotine altogether. Some damage to your lungs from vaping can heal or get better with medications. Over time, constant irritation to your lungs can lead to health problems (like asthma and COPD) that won’t go away. ENDS may be manufactured to look like conventional combusted cigarettes, cigars, or pipes. Larger devices, such as tank systems or mods, bear little or no resemblance to cigarettes.

However, the leading causes of nicotine poisoning are smokeless tobacco products (chew and snuff) and liquid nicotine that’s used in e-cigarettes. Chewing and snorting tobacco releases more nicotine into the body than smoking. Nicotine poisoning refers to the toxic effects of consuming nicotine, a chemical in all tobacco products. A recent increase in poisonings is due to liquid nicotine, a product in the popular e-cigarette.

Talk to your kids about e-cigarettes while they’re still willing to listen. In a recently published randomised trial of 886 subjects who were willing to quit smoking [100], the abstinence rate was found to be twice as high in the e-cigarette group than in the nicotine-replacement group (18.0% vs. 9.9%) after 1 year. Of note, the abstinence rate found in the nicotine-replacement group was lower than what is usually expected with this therapy. Nevertheless, the incidence of throat and mouth irritation was higher in the e-cigarette group than in the nicotine-replacement group (65.3% vs. 51.2%, respectively). Also, the participant adherence to the treatment after 1-year abstinence was significantly higher in the e-cigarette group (80%) than in nicotine-replacement products group (9%) [100]. Of note, another study indicated that although RANTES/CCL5 and CCR1 mRNA were upregulated in flavour/nicotine-containing e-cigarette users, vaping flavour and nicotine-less e-cigarettes did not significantly dysregulate cytokine and inflammasome activation [43].

Many popular e-cigarettes, like JUUL, are pod-based with single-use plastic cartridges containing nicotine. Generating even more waste are disposable e-cigarettes like Puff Bar, which are designed entirely for one-time use and have skyrocketed in popularity with a 1,000% increase in use among high school students between 2019 and 2020. And the 2021 National Youth Tobacco Survey found that more than 2 million middle and high school kids were current users of e-cigarettes, and that approximately 85% used a flavored product. The flavors that are the most popular with kids are fruit, candy, mint, and menthol.

E-cigarettes have not been approved by the FDA as a smoking cessation device, so consumers should exercise caution with this method. E-cigarettes themselves are very addictive, since most of them contain nicotine, and many who intend to use e-cigarettes to quit traditional smoking may find themselves continuing to use both products (dual use). Some e-cigarettes are disposable, while others are made to be reusable by refilling the device with liquid purchased separately. These liquids come in thousands of flavors and varying levels of nicotine; some may contain other compounds, like CBD or THC.

The FDA has approved two drugs that are synthetic forms of cannabis for this purpose. They also can be used to treat anorexia in people with acquired immunodeficiency syndrome (AIDS). Some people prefer to vape marijuana due to the milder smell, ease of use, affordability, and dose consistency. Some studies suggest that in some ways (lower carbon monoxide risk, for example) it may be safer than smoking.

As of July 2020, the sale of flavored e-cigarettes is prohibited in NYC. Vaping can cause lung injury and may affect lung health in other ways. Since COVID-19 can also affect your lungs, vaping may put your lungs at increased risk. E-cigarettes produce an aerosolized liquid (vapor) that usually contains nicotine and other chemicals inhaled by the user.

Since the end of 2019, it is illegal to sell vaping products to people younger than 21 years. Current evidence indicates that using e-cigarettes is dangerous, especially for young people and people who have never smoked. A 2016 study found that people who use or have used e-cigarettes are less likely to stop smoking altogether.

Instead, alarming evidence on adverse population health effects is mounting. You may have heard the benefits of vaping are that it’s safe or that it helps you quit smoking. United States federal law does not allow the sale of tobacco products to people under the age of 21. However, a major concern about vaping is its attraction for young people. These data overall indicate that the harmful effects of nicotine should not be underestimated. And to fast forward to today, we have taken final action, one way or the other, on more than 6.6 million of the almost 6.7 million products that were the subject of timely submitted applications. Roy S. Herbst, MD, PhD, deputy director for clinical affairs and chief of medical oncology at Yale Cancer Center and Smilow Cancer Hospital, says he would like to see more clinical trials on this subject. “Ideally, we would encourage someone who is already smoking to switch to e-cigarettes,” Dr. Herbst says. The order only applied to pods that go in refillable cartridge-based devices, and not to disposable e-cigarettes. The study, published on May 23, 2022, in Tobacco Control, is the first to look at the health care costs of e-cigarette use among adults 18 and older.

E-cigarette companies have promoted unsubstantiated health claims about their products as healthier than traditional cigarettes, when, in fact, e-cigarettes are uniquely dangerous for kids due to nicotine’s impact on their developing brains. This cross-sectional study highlights a high prevalence of e-cigarette use among adults in the US, particularly among young adults, in 2021. A striking finding is that 71.5% of those aged 18 to 20 years who reported e-cigarette use had no prior history of combustible cigarette use—this number is numerically higher compared with prior BRFSS data.

“Our finding indicates that health care expenditures for a person who uses e-cigarettes are $2,024 more per year than for a person who doesn’t use any tobacco products,” said lead author Yingning Wang, PhD, of the UCSF Institute for Health & Aging. Use of electronic cigarettes costs the United States $15 billion annually in health care expenditures – more than $2,000 per person a year – according to a study by researchers at the UC San Francisco School of Nursing. “Electronic Cigarette” means any device that can provide an inhaled dose of nicotine by delivering a vaporized solution [and] includes the components of an electronic cigarette including, but not limited to, liquid nicotine. This was a significant increase from 2016, when a national survey of high school and middle school students found that just 31% said that the availability of “flavors such as mint, candy, fruit, or chocolate” was the primary reason they used e-cigarettes. In addition, 81% of adolescents said they used flavors the first time they tried e-cigarettes.

In the UK, e-cigarettes are tightly regulated for safety and quality. They are not recommended for non-smokers and cannot be sold to people under 18 years old. So while they don’t have the conventional carcinogens we know are present at high levels in tobacco, we don’t know what some of the other things are going to do. This is due in part to the early introduction and widespread availability of e-cigarettes. Other countries have also reported increases in teen vaping, but the prevalence in the U.S. is notably high. By the early 2010s, as vaping devices became more sophisticated and flavors more appealing, their use among teenagers began to rise significantly.

The report also reveals a large increase in the concentration of nicotine in disposable e-cigarettes sold between 2015 and 2018. In 2015, disposable products on average contained 25 mg of nicotine per ml of e-liquid. This concentration increased nearly 60% to 39.5 mg/ml in 2018, increasing their addictiveness. Cartridge-based e-cigarettes contained even higher nicotine concentrations, with the most popular products having concentrations between 51 and 61 mg/ml, while the sale of nicotine-free cartridges was negligible. On the other hand, one potential challenge that the researchers uncovered in the results indicated that people without a cigarette smoking history – in other words, those who have only ever used e-cigarettes – might have a harder time quitting.

An e-cigarette, vape pen or other electronic nicotine delivery system (ENDS) heats a liquid of nicotine, flavoring, propylene glycol and other additives into an aerosol that you inhale through a mouthpiece. Vaping can cause breathing problems, organ damage, addiction and other conditions. Additionally, a study of adult smokers in Europe found those who vaped nicotine were less like to have stopped smoking than those who did not.

So when it comes to flavored e-cigarettes, we have this built-in concern about the popularity of those products with kids. But Congress did include mandatory considerations in the Family Smoking Prevention and Tobacco Control Act. And so, by law, when we get one of these applications, we have to consider the product’s impact on existing users of cigarettes and other tobacco products and on nonusers of the product.

It is estimated that 4.5% among all adults in the United States and 11% among adults aged used e-cigarettes in 2021. While adults who switch from smoking combustible cigarettes to using nicotine e-cigarettes reduce their tobacco-related health risks, e-cigarettes products are not harmless and sustain nicotine dependence. In surveys, more than half of adults who vape nicotine plan to quit. Cytisinicline, if approved, could be the first prescription drug to help people who are ready to address their addiction to e-cigarettes. Achieve plans to conduct an End-of-Phase 2 Meeting with the FDA later this year to discuss future clinical trial requirements to pursue an indication for vaping cessation.

Viewed as the catalyst for the modern-day tobacco control movement, the 1964 report remains a historic example of the importance of science-led public health action. E-cigarettes come in many shapes and sizes and can look like USB flash drives, pens, and other everyday items.
